Competition Forms
All the competition forms are in Microsoft Word 97-2003 (.doc) format for your convenience. The competition brochure is a complete, detailed list of everything you need to enter your dancers in our annual competition. It includes past winners, rules and regulations, entry forms, and more.
Deadlines for 2012 are January 22nd for the Team Deposit and February 1st for any changes prior to the publication of the Competition Schedule.

For your convenience, the medical forms and the liability forms are now posted here for you to download and print. They are in Microsoft Word 97-2003 (.doc) format for easy access. These forms must be turned in for each dancer participating. Without these forms, the team WILL NOT be allowed to perform. Please make sure these forms are turned into the competition office ASAP!
Medical Release Form and Waiver - Download
Please use the Fees worksheet in order to keep track of how much needs to be paid to the organization for your dance team. This is a very useful and recommended tool for all directors.
